Monitor - Your Reports Page visual improvements

ProcessCompletedOn should be added.  (Should only occupy 2 lines in the header so a rename may be appropriate) - It should display date and time.

Header should include Group Name in addition to the Group Number  (This is also a problem on other pages like 'Group Reports'.   This should be checked and evaluated on all pages)
