To assist with debugging issues, this table should have BOA Fields on it.  Our standard should be that every INSERT and every UPDATE done in our tools should always post to the proper fields regardless of whether a page to manage the table.  This standard should be added to code reviews or the appropriate point in the development cycle.

Business Case:  When debugging problems, identifying the process that created or updated a record enables accurate identification of what happened to put the data in the condition that needs investigation.
